Abstract: At 9:15 a.m. on January 18, 2024, Employee #1 was working inside a 7,000-gallon tanker trailer. Employee #1 was tasked with inspecting the damage of the interior of the tanker. Employee #1 was in the early stages of the tank inspection when he lost consciousness. A coworker noticed that Employee #1 lost consciousness, and s/he called the foreman to call 9-1-1. The coworker climbed into the tank to try and rescue Employee #1. The foreman arrived at the tank and realized the attendant wasn't on the ladder. The foreman grabbed an air hose and began to spray air into the tank. Employee #1 was killed.

Abstract: At 12:07 p.m. on December 23, 2023, an employee working as an equipment washer for a contract tank service was cleaning a bulk liquid waste tank that had contained acetic acid. The employee was found unresponsive inside the tank. The employee was transported to the emergency room and was pronounced dead eleven minutes after arrival. There was no visible trauma on the employee. The employee had died from possible carbon monoxide (CO) poisoning and/or asphyxiation.

Abstract: At 11:37 a.m. on December 4, 2023, an employee was working as a contractor performing repairs at a bulk petroleum storage facility. The employee was attempting to perform repair work inside a tank that had previously contained oil. The employee was using methylene chloride to degrease the floor inside the tank. During this process the employee stopped communicating and the employer attempted an entry rescue. The employee died due to chemical asphyxiation and acute exposure to methylene chloride.
