Abstract: At approximately 8:40 a.m. on September 17, 1990, Employee #1 was in a trench that was one block long by 4 ft wide by 8 ft deep to connect a 6 in. water line to a 10 in. water line. The walls of the trench were vertical except for the last 1 1/2 ft. The spoil pile was next to the edge of the trench. The west side of the trench caved in, burying Employee #1 above his knees. Two coworkers were trying to dig him out when the west side caved in again, completely burying Employee #1 and buring the coworkers to their waists. Employee #1 died of asphyxia.
