Abstract: Employee #1 was operating a standup Rider forklift, which was her normal job assignment. Employee #1 said that the forklift jerked forward, pinning her between the forklift and some stationary metal shelving. There were no witnesses. Other employees heard Employee #1 yell for help. Employee #1 complained of abdomen and upper leg injuries and 911 was called to assist and to transport her to the hospital. The injuries were not considered to be life-threatening at the time of the hospital transport, but Employee #1 died five days later of blunt trauma of perforated sigmoid colon, peritonitis, and probable sepsis.
