Abstract: In June of 1988, a one inch pipe and fittings manifold was removed from a condensate dump line and was placed on the ground at a gas production facility. The manifold was used to connect the condensate wall dump valve to the dump line at about 11 a.m. on March 13, 1989. Employee #1 re-installed the rusted manifold on the 3,200 foot, 470 psi gage pulsation bottle. Employee #1 opened the dump valve and proceeded to vent condensate vapor and natural gas from the pulsation bottle. Suddenly the one inch pipe broke, swung over and struck him on the left side of the head. Employee #1 was transported to St. Anthony Hospital in Oklahoma City where he died on March 14, 1989.
