Abstract: An employee worked as a corrections officer. On November 22, 2021, the employee was identified as a close contact of a COVID-19 positive individual and was sent home (it is not specified in the narrative if the individual was an inmate or coworker). On December 2, 2021, the employee contacted his employer and reported he had been experiencing COVID-19 related symptoms and was still symptomatic. The employee took a home COVID-19 test and the results were positive. The employee did not send a copy of the test results to the employer. The employer told the employee they required a test from one of the local community-based test sites and the employee said he would get that done. On Sunday, December 5, 2021, the employer received notification that the employee died sometime on Saturday, December 4, 2021. The cause of death was from acute pulmonary thromboembolus and chronic organic heart disease presumed to be related to COVID-19, but the employer did not have a record of a positive COVID-19 test. The employee had not been vaccinated.
