Abstract: At 3:15 a.m. on March 25, 2021, Employee #1 was loading stock product pieces into the feed tray of a custom-designed stencil machine, which stamps the company name on the product and presses a rubber seal into place. The machine was enclosed by a plexiglass guard around its perimeter, but the top was unguarded. During operation, the machine malfunctioned and failed to stamp the product. While troubleshooting the issue, Employee #1 placed his hand into the point of operation. Both sensor switches closed, causing the machine to cycle and crush his fingers, resulting in amputation. The employee was hospitalized.
