Abstract: At approximately 3:45 p.m. on September 12, 2014, an employee was performing maintenance. The employee was cycling the golf shaft straightener to position A feed system, which is located at the start of the machine. Shafts are loaded by the operator where they travel by gravity to a slow rotating lift device. This lifting device has two PVC plastic holders designed to hold each end of a shaft. There are multiple PVC plastic holding stations located on the feeder to allow continuous operation. This lift travels or revolves to allow one shaft to be loaded at a time into the straightener. One of the PVC plastic holders was broken. The maintenance tech observed the machine and found the broken plastic holder to be on the bottom of the revolution or below the machine. The employee used the right hand to hold the PVC plastic holder as he cycled the machine to get it to a position to be repaired. During the rotation of the transfer device, the employee's finger was caught between the plastic holder device and a structural brace of the machine. The right finger was amputated. The employee was treated but not hospitalized.
