Abstract: At approximately 12:45 p.m. on February 9, 2021, an employee returned from lunch and was told by maintenance that the machine was ready for threading operations. The employee began operating the Dubin #32 Roll Thredder set up on 1-318" X 36" rowldbars running at approximately 100 RPM speed. The employee began to thread a bar; he flipped the threaded side of the bar to the "V" shaped opening with the cut side spinning and exposed. The employee turned into the bar to take the bar off of the threader and his shirt got caught in the threaded/cut side of the bar behind the "V" away from the machine. The spinning bar wrapped the injured employee's shirt and pulled him in the direction of the bar. The bar popped up and then the injured employee reached over the bar with his left hand and the bar snapped back into the "V" catching his left hand in between the "V" and the bar. The employee incurrred a fracture of the middle left finger, an amputation of the left index finger, and lacerations of the top of the left hand. The employee was not hospitalized.
