Abstract: At approximately 8:30 a.m. on December 1, 1999, Employee #1 was helping two other maintenance mechanics repair an inclined conveyor. Employee #1's left hand was on the chain and sprocket. When the conveyor started, catching his thumb and three fingers. His left thumb was partially amputated, his index and middle fingers sustained multiple fractures, and his ring finger was lacerated. He was hospitalized for three days and underwent three surgeries on his hand. The conveyor was not de-energized or locked out prior to it being repaired.
