Abstract: Employee #1 was working at a trim press when the machine jammed. A work table separated Employee #1 and his foreman, who was located on the side of the table near the EMERGENCY STOP button. Employee #1 asked the foreman to push the STOP button so that he could clear the jam, but he refused because the company had no lockout procedure or policy . Employee #1 then tried to clear the jam with the machine running when his right hand became caught in the press, amputating the upper portions of all five fingers. The employer was cited for a serious accident-related violation of T8CCR 3314(a), for not establishing and implementing the required lockout procedure. Other general and serious regulatory violations were observed at the time of the inspection and the employer was cited accordingly.
