Abstract: These are two separate incidents caused by one patient. At approximately 5:30 p.m. on December 4, 2018, Employee # 1, a full time licensed practical nurse was assigned to perform one on one duties for a psychiatric patient. At the time of the incident, Employee # 1 was in Psychiatric Ward C observing the activities of the patient by standing in the ward's hall way. The patient was in his room. The patient became agitated at Employee # 1 and began to ask him "where his black book was". After a brief attempt to de-escalate, the patient charged Employee # 1. The patient began to punch him and opening and closing a door on his head. Employee # 1 was trapped and a Code Gold was called. The sheriff's department arrived at the ward and subdued the agitated patient. After the patient was subdued, management called 911 and fire and paramedics and transferred Employee # 1 to the hospital. At the hospital, he was observed for possible head injuries. Employee # 1 was diagnosed with receiving contusions to the head and released from the emergency department of the hospital the same day, at approximately 10:00 p.m., approximately an hour after being seen. Management reported the accident to the state OSHA office on December 4, 2018 at approximately 8:43 p.m. The Division arrived at the hospital on January 4, 2019 at approximately 10:30 a.m., to investigate the accident. At 5:31 p.m. on January 19, 2019, a psychiatric patient went to the nursing station in Psychiatric Ward B to ask for medicine. Employee # 1, a nurse, asked the patient to go to the other side of the nursing station to wait for the medicine, while she prepared the medication. The patient became agitated and started banging the door. Employee # 1 was overpowered by the patient and began to punch her. The Behavioral Response Team responded to the incident and got the patient off of Employee # 1. She was taken to the emergency room and was treated and released with a contusion to the forehead. She was released from the emergency room the same day. Management reported the accident on January 18, 2019 at 4:30 p.m. The Division opened up an investigation on February 23, 2019. The dates are not in chronological order and do not make sense as presented. It would be reasonable to assume that the accident was reported after the date it occurred, January 18, 2019 but before it was investigated on February 23, 2019.
