Abstract: At approximately 4:00 p.m. on May 30, 2018, Employee #1 was driving a Kenworth Model W900 water truck in a southward direction, down a declining grade, applying water to Forest Service Road 12. The employee tried to shift the trucks manual transmission into a higher gear, but he shifted into neutral and the truck began gaining speed. He was unable to shift the transmission into gear, so he tried to use the service break to stop the truck. The trucks brakes were unable to stop the vehicle. Employee #1 traveled approximately one mile before coming upon a corner in the road. The vehicle was now traveling at 35 mph. He negotiated the corner, but the employee lost control approximately 450 feet beyond the corner. The truck left the gravel roadway, and traveled through dense brush, coming to a stop on its passenger side after striking a tree. Employee #1 suffered a laceration to his head and sustained five broken ribs. He was hospitalized for his injuries.
