Abstract: At 12:35 p.m. on January 24, 2018, two employees were in an aerial lift (Genie Z-45/25, equipment Number KS4K78) performing tree-trimming work with a chain saw. The two employees were working approximately 35 feet above the ground. Employee #2 was cutting a section of the tree with a chain saw and Employee #1 was operating the lift and monitoring the tree-trimming operation. The section of the tree being cut fell towards the platform and struck Employee #1 on the head. Employee #1 was partially ejected from the platform and became unconscious. A coworker climbed up the boom with a rope to help lower unconscious employee #1 to the ground. Employee #2 and the co-worker did not know how operate the elevated platform controls and climbed down the boom. Employee #2 lost grip while climbing and fell approximately 10 feet from the boom to the ground. Employee #1 was comatose and transported to a medical center and hospitalized. Employee #2 sustained minor neck and back pain and lacerations to his left elbow and upper lip and was hospitalized.
