At 8:30 a.m. on June 22, 2018, an employee was assisting coworkers change out a die/mold on a piece of machinery. Normally the process involved the use of a die cart, but a forklift was used instead. During the change out, one coworker oper ated the forklift to hold a platform in place, while the employee and two cowork ers removed the die/mold from the machine. The die/mold being replaced was to be rested on the platform being held by the forklift. The employee and his cowork ers first pushed the die/mold out of the machine and were pulling it onto the pl atform when it slid off the open side of the platform. The employee fell backwa rds and the die/mold fell onto his legs. The employee was pinned between the app roximate 6,000 pound die/mold and the floor. The employee's legs were crushed an d he was hospitalized, where he underwent surgery.
