Abstract: At 6:30 p.m. on June 7, 2018, Employee #1 was holding a bundle of reinforcing steel on a truck trailer to allow the forklift forks to back out of the load after placement. The bundle consisted of 96 pieces, weighing one ton, and were 20 feet in length. As the forks pulled out of the load, the employee shoved the load, causing it to roll off of the truck trailer and onto the employee's foot. The employee sustained a fracture of the right foot, a laceration to the right heel, and an avulsion to the three small toes. He was hospitalized for his injuries.
