Abstract: At 7:30 a.m. on May 1, 2018, Employee #1, employed by a home builder, was preparing to work on the roof of a two-story residential structure. He stepped onto the roof, which had a 3.5:12 pitch, intending to install toe boards for fall protection. Frost had created a slippery surface on the roof decking, and Employee #1 slipped and fell, a fall height of 16 feet. Emergency services were called, and Employee #1 was transported to the hospital, admitted, and treated for fractures to his pelvis and left eye socket.
