OSHA Log Data Collection

Welcome to OSHA's Log Data Collection site. Public Law 91-596 (the Occupational Safety and Health Act of 1970) requires certain employers within specific industries to annually submit information to OSHA, describing any work-related injuries and/or illnesses that have occurred within their workforce.

Under Public Law 91-596, all establishments that receive a request for OSHA Work-Related Injury and Illness Data must submit data within 30 days. If your establishment received a request for data, your options are: you may complete a paper copy and submit it by mail or fax OR you may submit data electronically using this secure website.
