Weekly Fatality/Catastrophe Report

This table contains the weekly summaries of fatalities and catastrophes resulting in the hospitalization of three or more workers. Employers must report these incidents to OSHA within eight hours. The summaries below include only preliminary information, as reported to OSHA Area Offices or to States which operate OSHA-approved State Plans. The fatalities listed here include only those that initially appear to be work-related, but exclude fatalities that do not appear to be work-related, such as an apparent heart attack of a sedentary worker. OSHA investigates all work-related fatalities and catastrophes. After OSHA's investigation is complete, these reports will be updated with inspection results and citation information.

9/19/2011 The Indian Club, Irons, MI 49644 Worker suffered burns and died from asphyxiation after a storage facility fire.
9/21/2011 Livengood Excavators, Palmerton, PA 18071 Worker was killed when his dump truck overturned, rolled down a hill and landed on top of him.
9/21/2011 Stevens Electric & Pump Service, Inc., Kennebunkport, ME 04046 Two workers died from overexposure to hydrogen sulfide while inside a sewer tank.
9/22/2011 Reproduction Services, Inc., Stillwater, OK 74075 Worker, on the roof of a barn he was building, died when he fell through a skylight, hitting the floor below.
9/29/2011 Integral Bldrs LLC, Dewitt, MI 48820 Worker building an addition on a barn was electrocuted after touching a 4,800-volt powerline.
9/30/2011 Commercial Construction, Inc., Kearney, NE 68924 Worker using a dozer to fill a lake with sand drowned when the sand could not support the equipment's weight.

9/30/2011 Steve Wolota General Contractor, McKees Rocks, PA 15136 Worker performing roofing work died after falling more than 10 feet to the ground.
9/30/2011 Greenstar Recycling, Northampton, PA 18067 Worker performing maintenance on a baler was killed when falling cardboard caused him to fall back onto a protruding machine part.
10/3/2011 TNK & Servives, Inc., Sonora, TX 76950 Worker checking an air leak underneath a truck was killed when the vehicle ran over him, dragging him 80 feet.
10/4/2011 US Army Environmental Division, Trinidad, CO 81082 Worker was injured during a vehicle accident.
10/5/2011 All Tree, Brenham, TX 77833 Worker using a pole saw to trim trees was electrocuted when the saw touched a powerline.
10/5/2011 Power Source, Zionsville, IN 46077 Worker died after falling 22 feet when an open basket door became caught.
CATASTROPHES - MULTIPLE WORKERS HOSPITALIZED
10/3/2011 Southwest Enterprises, Goshen, IN 46526 Four workers at a fertilizer plant were injured when a tank holding 2,000 gallons of ammonia exploded.
