The table below provides weekly totals of fatalities and catastrophes, tabulated by region. Employers must report these incidents to OSHA within eight hours. The data includes only preliminary information, as reported to OSHA Area Offices or to States which operate OSHA-approved State Plans.

* Note: Corrections were made to these totals to correct errors in previously posted reports, including one for the week ending 5/21/2011, for a double fatality that occurred on 5/18 in Las Vegas, New Mexico, that was originally posted and counted as having occurred in Las Vegas, Nevada. Two other corrections are made here to account for errors in the State Plan FY totals for Regions V and VI in the weeks ending 7/16 and 7/23.
