29 CFR 1926.501(b)(1): Employees on walking/working surfaces (horizontal and vertical surface) with an unprotected side or edge 6 feet (1.8 m) or more above a lower level were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: (a) On or about July 8, 2013, two employees engaged in construction activities on an existing communications tower near Belden, North Dakota, were wearing, but not using, personal fall arrest systems when exposed to a fall hazard of approximately 225 feet. The employees fell approximately 225 feet from the tower to the ground below, sustaining fatal injuries.
