Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee in that employee was exposed to the hazard of arc flash and arc blast: a) 930 N. Riverview Dr., Totowa/Weston Bakeries' circuit panel: An employee was exposed to an arc flash while installing a circuit breaker on a bus energized to 480 Volts. The employer proceeded with work on live parts which was not placed in electrically safe working conditions, without performing a Flash Hazard Analysis and without using safety related work practices based on the determined hazard such as the use of PPE appropriate for protection from an arc flash, on or about 03/16/06. Note: Among other methods, feasible and acceptable abatement methods to correct this hazard are: 1. Perform Flash Hazard Analysis to determine the safe working distance from the panel. 2. Use appropriate tools rated for hazards associated with working on energized circuits. 3. Use the appropriate PPE for protection against flash hazards such as, but not limited to, fire retardant gloves and coveralls.
