    Speaker Notes:

    The SGE application packet is about 25 pages long, however, there are only 3 documents that need to be completed and returned to the SGE Coordinator. They are:

    VPP SGE Eligibility Information Sheet – provides information for your name, employer, when your site became VPP, your safety and health experience (safety, health, PSM, construction, etc.) Newly added… signature of manager which implies concurrence with applicants qualifications, US citizenship, etc.

    Optional Application for Fed’l Emp (OF-612) OR Current resume– you may submit one or the other. If you chose the OF 612, please answer all questions and sign it and date it. If submitting a resume, please ensure that it is current.

    Confidential Financial Disclosure Report (OGE Form 450) - this form lists your assets/income; liabilities, etc. This form is very important in that it is used to ensure that there are no financial conflicts between you and the site you have been requested to audit. Example….if you work for Coca-Cola and you have stocks or dividends in the company, you will not be approved to participate on that Coke audit.
