Maine Healthcare Association, OSHA, and nursing home and personal care
facilities aim to reduce injuries and illnesses, and to streamline the
implementation of industry regulations and procedures. Information and
training will focus on hazard recognition and OSHA standards.
Results: From May 2002 to February 2004, 45 Health Care facilities in
Maine participated in this Partnership agreement. OSHA conducted 8
training sessions which were completed throughout the state and
covered various safety topics. |

Partnership Overview:
The Associated General Contractors of America and OSHA partnered to improve safety and health at construction sites. Goals included annual reductions of 3% in injuries, illnesses, and fatalities, implementation of effective safety and health programs at partner sites and the provision of training to management, supervisors and employees.
Partnership Results:
This partnership impacted 7 employers and 682 employees. For the combined partnership participants, 1300 training sessions were conducted by non-OSHA staff, with a total of 516 employees attending the trainings and a total of 2200 self-inspections were conducted. For all CHASE participants combined, when comparing the 2006 rates versus the Construction Industry 2005 BLS data, the CHASE participants TCIR is 6.4% below the 2005 BLS rate.
Dimeo Construction's TCIR and DART, respectively, were 29% and 9% below the national average, when compared to the Construction Industry's 2005 BLS data. |

Overview: Participating employers establish or improve their
safety and health management systems, with focus on preventing employee injuries during
patient handling.
Results: After becoming a participant in this Partnership, one
nursing home facility completed site surveys with a focus on patient handling. As a
result, the Resident Handling Incident Rate decreased from 20.5 in 2002 to 6.6 in 2003.
Additional nursing homes were added in 2004 and by 2005, 15 employers and 3, 278 employees
were covered by the Partnership. |

Partnership Overview:
The Associated General Contractors of America Massachusetts Chapter and OSHA partnered to improve safety and health at construction sites. Goals of the partnership included: reducing by 3% annually the number of injuries, illnesses, and fatalities affecting participant employers, and implementation of effective safety and health programs at partner sites with a provision of training to management, supervisors and employees.
The partnership continued to focus on construction industry hazards such as: falls; struck-by; electrocutions; and caught-in and caught-between.
Partnership Results:
Results from the OSP's 2010 annual evaluation included the OSP offering over 533 safety and health training sessions consisting of 2,600 hours to 5,504 workers. The OSP's participating companies conducted 13,399 self-inspections which resulted in the identification and abatement/correction of 15,688 hazards and/or violations.
The participants of the OSP attained an aggregated Total Case Incident Rate (TCIR) of 1.8, and a Days Away, Restricted, and Transferred (DART) rate of 0.3 which were 57 and 82 percent below 2008 Bureau of Labor Statistics (BLS) respectively for the Nonresidential building construction. |

Overview: This partnership involves the renovation of the former
Hartford, Connecticut Civic Center Mall site with a mixed-use development encompassing
residential, retail, and public access and parking features. The partners are Turner Construction
Company, Construction Trades Council, and the Building Trades Employer Association. The specific
goals include: eliminating the four primary construction hazards (falls, electrical, caught
in/between, and struck–by); minimizing employee exposure to unprotected walking/working surfaces
(horizontal and vertical) that are six feet or more above a lower level; and keeping Turner
Construction Company's days away, restricted work activity, and job transfer (DART) rate at 25% or
more below the national average.
Results: The project, now known as Hartford 21, is scheduled for completion in the Fall of 2006,
with the final beam of the 36-story, 262-unit luxury residential tower (the tallest in New
England) having been lowered in October 2005. Employees covered by the partnership numbered
approximately 285 during construction in 2005. |

Overview:
E.D. Swett, Inc., the New Hampshire
Department of Environmental Services, Consultation Program and the
Occupational Safety and Health Administration of the United States Department
of Labor (OSHA) Concord Area Office (CAO) mutually recognize the importance of
ensuring a safe and healthful work environment in the construction industry in
the state of New Hampshire.
This partnering agreement, which has been developed for the Granite Street
Bridge Project on Granite Street, in Manchester, New Hampshire, will be
limited to E.D. Swett, Inc. and associated project employers who have agreed
to participate. E.D. Swett, Inc. will request specialty or trade contractors
to join the partnership when they start working on the site. The partnership
will provide benefits to E.D. Swett, Inc. and the specialty or trade
contractors, which include, among others, special recognition from OSHA and
priority in compliance assistance programs.
The E.D. Swett, Inc. project consists of the rebuilding of the Granite Street
Bridge. It is anticipated that at the peak of the project, E.D Swett, Inc. and
its approximately 10 subcontractors will employ approximately 25 trade
employees on this project.
Results:
During the most recent evaluation period,
this partnership impacted one employer and 25 employees. Eight training
sessions were conducted by OSHA staff with a total of 25 employees attended
providing over 900 training hours. Every person on site received the OSHA
10-hour training, as well as, weekly toolbox trainings. In addition, annually
there is a three day Management Safety training session, and a company
training day for all employees. OSHA staff provided technical assistance to
the partner. Their TCIR and DART rates, respectively, is 63 and 31 percent
below the National BLS Averages for their industry. |

limited to the Cobb Hill/Pro Con Construction Joint Venture, LLC, and specialty or trade
contractors at the site. The primary goals of this partnership are: (1) Reduce injuries illnesses
and fatalities resulting from falls struck-by, caught-in between, and electrocution hazards and
trenching by 5% (2) Decrease workers compensation cost and number of OSHA citations.
Results: During the most recent evaluation period, this partnership
impacted 21 employers and 250 employees. Four training sessions were conducted by OSHA staff with
a total of 250 employees attending these trainings. Among the training conducted at Cobb Hill were
fall protection, steel erection, precast concrete and excavations. In addition, 81 employees
attended an OSHA 10-hour training. Also, throughout the duration of the evaluation period,
eighty-four (84) self-inspections were performed and 99 hazards were abated. |

Partnership Overview:
This partnering agreement for
the project located at T.F. Green Airport in Warwick, RI, was limited to the
Skanska USA Building Inc., and specialty or trade contractors at the site.
Skanska USA Building Inc. will request specialty or trade contractors to join
the partnership at the time they start working on the site. The partnership
provided benefits to, Skanska USA Building Inc. and the specialty or trade
contractors, which included, among others, special recognition from OSHA and
priority in compliance assistance programs.
The Skanska USA Building Inc project components consisted of an in-line EDS
Baggage Handling System, Security Screening Checkpoint Expansion,
Administrative Office Expansion, Accelerated Lobby Infill and Pre-Screening
Concessions, Security Bypass Sloped Walkways, Concessions Expansion, Ticket
Counter Expansion and Bag Belt Consolidation. The project incorporates
approximately 93,000 square feet of new construction and 244,000 square feet
of renovated space at a projected cost of approximately 80 million dollars.
Partnership Results:
At the peak of the project,
Skanska USA Building Inc. Partnership will impact approximately thirty-five
(35) specialty or trade contractors, and approximately one hundred and fifty
(150) trade employees on this project. The major goals of the partnership
wereto reduce by 5% annually the number of injuries, illnesses, and fatalities
affecting participating employers, with an emphasis on reducing injuries and
fatalities resulting from falls, struck-by, caught-in/between, electrocution
and trenching. Decrease worker compensation costs and the number of OSHA
citations for this project, and sharing best practices. The results of the
annual evaluation show the TCIR and DART rates respectively, is 66 and 55
percent below the BLS National Average for their industry. There were over 250
safety training sessions with 265 employees in attendance, that included such
topics as First Aid (3 hours) and CPR (2 hours). The site also had 240 self
inspections with 35 hazards recognized and abated. |

Partnership Overview:
This partnering agreement
covered the construction of Connecticut Center for Science and Exploration
located in Hartford, Connecticut. The main partner is The Whiting-Turner
Contracting Company and this partnership covered any specialty or trade
contractors at the site. Specialty or trade contractors will be requested to
join the partnership at the time they start working on the site. The
partnership provided benefits to The Whiting-Turner Contracting Company, and
the specialty or trade contractors, which included, among others, special
recognition from OSHA and priority in compliance assistance programs.
The Connecticut Center for Science & Exploration will consist of a three level
cast in place parking garage with a multi level museum structure constructed
on top of the plaza level. At the peak of the project, The Whiting-Turner
Contracting Company and its approximately twenty (20) specialty or trade
contractors will employ approximately two hundred (200) trade employees on
this project.
Partnership Results:
This partnership affected about 33
employers and 190 employees. During the first year of the partnership, over
175 Safety Orientations were given to every worker on their first day of work
on the project at Whiting-Turner. Finally, a HAZWOOPER 8 hour refresher course
conducted by Field Safety Corporation.
Whiting Turner developed a safety committee to include one
non-Forman/non-safety professional to meet bi-weekly. The committee discussed
jobsite hazards and recommendations on how to correct the hazards. Whiting
Turner would take the lead to correct the hazards immediately. The contractor
conducted over 626 inspections, and 64 hazards found were abated during this
rating period. |

Overview:
To facilitate the goal of reducing
occupational related fatalities and serious injuries within the construction
industry, OSHA and Turner Construction Company have agreed to enter into a
cooperative partnership agreement which will effectively implement all facets
of jobsite safety and achieve self-compliance through cooperative efforts from
labor, management, and OSHA.
This partnership is designed to address hazards within the construction
industry, and to promote and recognize those jobsites controlled by a
contractor that have demonstrated an effective safety and health program. The
Partnership agreement is an effective tool for ensuring safety at the North
Pavilion / Cancer Hospital for Yale-New Haven Hospital.
Turner Construction Company will serve as the general contractor for the $250
million North Pavilion / Cancer Hospital for Yale-New Haven Hospital in New
Haven, Conn. The hospital will be the most comprehensive cancer care facility
between Boston and New York City. Completion of the tower is scheduled for
2010.
Results:
During the most recent evaluation period,
this partnership impacted 50 employers and 771 employees. Two training
sessions were conducted by OSHA staff and 50 by non-OSHA staff, with a total
of 600 employees attending these trainings. Every person on site received the
OSHA 10-hour training. In addition, 14 Turner workers and 31 subcontractor
workers received the OSHA 30-hour course. |

Overview: The Project consists of a restoration
project for the old National Guard Armory whose footprint encompasses three
city blocks. The building is a National Historic landmark built in 1907. The
building is considered to be one of eleven most endangered building in the
United States and is currently occupied by two Rhode Island State Agencies.
The Project includes, but is not limited to, two 180 foot stone towers, two 90
foot stone balconies and a 60 foot stone chimney. The building is scheduled to
be restored in phases and is scheduled for completion in early 2008.

The Middletown - Norwalk Transmission Project is a system of higher capacity power lines required
to address Connecticut's growing energy needs. The Project has been broken into four segments. The
UI and B & V's portion of the project will consist of the erection of the new Singer Substation in
Bridgeport, the construction of 6 miles of underground concrete-encased duct line, and the
connection of underground lines to the Singer and Pequonnock substations. |

The Project consists of two four-story buildings with a total of 100,000 square feet of classroom
and office space. The buildings will consist of structural steel frames, masonry block and brick
exteriors, with curtain walls and pitched copper roofs. The expected completion date for the two
buildings is September 2008. |

provide benefits to Cianbro Corporation and the specialty or subcontractors,
which include, among others, special recognition from OSHA and priority in
compliance assistance programs.
The Project consists of a seven story hotel, a four story parking garage and a
135,000 square foot gaming facility. The expected completion date for the two
buildings is December 2008. |

This partnering agreement involves Consigli Construction
Company, Inc. and the project is the construction of the Combat Training Pool located at Naval
Station Newport and Officer Training Command in Newport, Connecticut. The Project consists of a
naval combat pool facility that is being built to accommodate the necessary training and support
functions for naval aquatic survival and officer accession training. The facility will include a
25 yard 8 lane pool with a 3 meter diving platform to be used for basic swim qualifications and
man overboard training; male and female locker and shower rooms; observation office; first aid;
laundry and storage rooms. The 11,280 square foot training facility will provide a current and
secure training and support location to maximize effective naval combat and survival training. The
primary function of the facility is combat training. The 4.8 million dollar project is scheduled
for completion in October 2008. The goals for this partnership include: 1) reduce workforce
fatalities, serious injuries, and illnesses and provide a safe work environment for employees at
the Project; 2) reduce by 5% annually the number of injuries, illnesses, and fatalities affecting
participating employers, with an emphasis on reducing injuries and fatalities resulting from
falls, struck-by, caught-in/between, electrocution and trenching; and 3) decrease worker
compensation costs and the number of OSHA citations for this project. |

Overview: This partnering agreement involves
Consigli Construction Company, Inc. which began a 24 month courthouse
construction project. The 90,000 square foot structure will incorporate a
total of seven courtrooms. A structured parking garage below the building will
accommodate 60 staff vehicles and a state-of-the-art security system will
control access to all parts of the building. The goals for this partnership
include: 1) reduce workforce fatalities, serious injuries, and illnesses and
provide a safe work environment for employees at the Project; 2) reduce by 5%
annually the number of injuries, illnesses, and fatalities affecting
participating employers, with an emphasis on reducing injuries and fatalities
resulting from falls, struck-by, caught-in/between, electrocution and
trenching; and 3) decrease worker compensation costs and the number of OSHA
citations for this project.
Results: From inception of the partnership to the end of the evaluation period
in October 2009, 7 of the 8 employers that participated in the partnership
implemented effective safety and health management systems. Additionally, all
workers received the OSHA 10 hour training course and 30 hours of other
training that coverer the following topics; Mast Climber Training, Cold and
Hot Weather Stress, Respiratory Protection, Electrical Hazards,
Excavation/Trenching Safety, Fall Protection, Scaffolding Safety, Mobile
Equipment and crane safety. During the 19 month project the site experience 2
lost workdays incidents and achieved a Total Case Incident Rate (TCIR) of 2.4
and a Days Away, Restricted and Transfer (DART) rate of 0, which was 55% and
100% below the 2006 Bureau of Labor Statistics' (BLS) national average. |

Partnership Overview: To advance the importance of ensuring a
safe and healthful work environment in the construction industry in the State
of Connecticut, the partnership between Petra Construction Corp, the BAO and
CONN-OSHA has been established to formalize a cooperative effort between these
entities, encourage companies to improve their safety and health performance
voluntarily, provide methods to assist them in their efforts, and recognize
companies with exemplary safety and health programs.
This partnering agreement was for the construction of a 12,000 square foot
residence located in Guilford, Connecticut. The project consisted of the
erection of a 2.5 story wood frame residence. This Partnership provided for
training and education for up to 30 subcontractors. Workers for these
subcontractors are exposed to the four recognized hazards within the
construction industry which are falls, electrocution/electrical shock,

Brewer Professional Center worksite in Brewer, Maine. The Partnership, in

between hazards. The purpose of the Partnership was to provide training in
recognizing the four hazards in the construction industry, develop methods of

workplace. The partnership explored ways to pursue training and education for
up to 15 subcontractors.
The project consisted of the construction of a three story 136,000 square foot
building with a full basement and a mechanical penthouse on the roof. The
building will have a structural steel frame, metal panel and curtain wall
exterior, and a flat membrane roof. The building will house a cancer treatment
center that includes chemotherapy infusion bays, CAT scan facilities and three
linear accelerators. |

Overview: Recognizing the importance of ensuring a
safe and healthful work environment in the construction industry in the
Commonwealth of Massachusetts, OSHA and Erland Construction, Inc. signed a
comprehensive safety and health partnership on July 31, 2008.
Goals of the partnership include: reducing the Total Case Injury Rate (TCIR)
and Days Away Restricted Transfer (DART) rates by 5% annually compared to, the
BLS data for General Contractors - Non-Residential/Industrial Buildings SIC
and reducing injuries and fatalities. The partnership will focus in particular
on construction industry hazards such as: falls; struck-by; caught-in;
caught-between, electrocutions and trenching and excavation and exposure to
silica during construction activities.
The Sunrise of Franklin Senior Living Facility project in Franklin,
Massachusetts, involves the construction of a senior living facility
consisting of 79 living units with a complete food service and dining
facility. The building structure consists of four stories with structural
steel frame, metal deck with concrete slabs. The exterior walls will be metal
stud and dens-glass sheathing with cementitious horizontal siding. The roof
consists of membrane roof on the main roof, shingles at the mansards and metal
roof at the porch and entrance. There will be two elevators installed that
will service all four floors. The resident units will be serviced by
through-wall HVAC units. The building is designed with a complete fire
protection system including a fire pump and will be equipped with an emergency
generator for back-up power.
Founded in 1977 in Burlington, MA, Erland Construction, Inc. provides Program
Management; Construction Management; Design/Build; and General Contracting
services to corporate, institutional, educational, industrial, residential,
and commercial development clients.
The project is scheduled for completion during September, 2009. |

The Occupational Safety and Health Administration (OSHA)'s Providence Area Office, Rhode Island Consultation Program and Dimeo Construction Company signed a partnership agreement to develop a working relationship that results in decreased serious injuries; illnesses; and fatalities for participating contractors and improvement of existing safety and health programs.
Goals of this partnership included:
Reducing workplace injuries and illnesses by establishing a workplace where recognized industry hazards are minimized and controlled through the creation of a pro-active health and safety culture. Provide safety and health training to all employees. The agreement targeted the prevailing hazards in the construction industry; falls electrocution/electrical shock, struck-by and caught in between. Improvements in these areas were to be accomplished through demonstrated leadership and the development and implementation of effective health and safety management systems.
The project consisted of reconstruction of the 120,495 square foot hotel. The south wing of the hotel will precisely replicate the original building which dates back to 1908. In addition, there will be a 31,928 foot internal, underground parking garage.

The second phase began in September 2009 and included the construction of a new 150, 000 square foot building. The third phase began in August 2011 and included interior demolition of the existing east wing. Construction of a new 22,000 square foot south wing and renovation of the existing west wing was the final phase of the project. |

Parsons Hall primarily houses the chemistry department. It is estimated that 116, 000 of the 146, 000 gross square feet will be renovated as part of this project. Partnership Overview:
Black & Veatch was selected to modernize the Grand Station Avenue Switching Station, a component of the United Illuminating Company transmission network that serves more than 324,000 electricity customers in southwestern Connecticut. Black & Veatch provided complete engineering, procurement and construction services for the approximately $60 million modernization project that involved developing a new, indoor switching facility that uses gas-insulated switchgear (GIS) technology. The project also involved the reconfiguration of four overhead and three underground power transmission lines. The project was scheduled for completion in 2012.
Partnership Goals:
To provide a safe and healthful work environment for workers on the Station construction project and to help prevent fatalities and serious accidents and injuries within the industry through increased training, implementation of best work practices, enhanced safety and health programs.
Partnership Results:
Due to close monitoring and safety inspections conducted by Black & Veatch's Safety Director and OSHA's Compliance Safety and Health Officers, the project was free of recordable injuries and illnesses or days away from work. A total of sixty employees were trained for the duration of the Partnership, with over eighty-seven manhours spent on training. The project was completed on budget and approximately sixteen months early than projected. |

The Project consisted of an administration building, a classroom building, a waste water treatment building, and the renovation of the existing 42,000 square foot field house. The Project also included the demolition of the existing structures and an extensive site improvement package. The Project value was estimated at about $56 Million.
